  • Salary: AUD $82,740– $98,819 per annum + Superannuation and applicable benefits

Key Responsibilities

  • Conduct assessments to identify clients' social, emotional, psychological, family and practical support needs.
  • Develop and implement appropriate intervention and support plans in collaboration with clients, families and relevant healthcare professionals.
  • Provide professional counselling, psychosocial support, advocacy and appropriate referrals to community and support services.
  • Support clients and families to understand and navigate healthcare, community and social support systems.
  • Work collaboratively with multidisciplinary teams to contribute to comprehensive, person-centred care.
  • Maintain accurate, confidential and timely client records, case notes, assessments and reports in accordance with professional and organisational requirements.
  • Identify risks, safeguarding concerns and barriers affecting a client's wellbeing and escalate matters appropriately.
  • Assist clients experiencing complex social, family, financial, housing, mental health or other challenges by connecting them with appropriate services and resources.
  • Communicate effectively with clients, families, carers, healthcare professionals, government agencies and community organisations.
  • Participate in case conferences, multidisciplinary meetings, care planning and service coordination activities.
  • Provide professional advice and contribute to education, prevention, community development and service improvement initiatives where required.
  • Maintain professional standards, ethical practice, confidentiality and compliance with relevant legislation, policies and Social Work professional requirements.
