About This Opportunity

Country Health Connect is seeking a compassionate and adaptable Registered Nurse to join the community nursing team in Naracoorte, South Australia. This casual position provides an opportunity to deliver meaningful healthcare services across community and home-based environments while supporting individuals and families to achieve improved health outcomes.

As a Community Health Nurse, you will work independently and collaboratively within a multidisciplinary team to assess client needs, coordinate care, provide health education, and contribute to health promotion initiatives across the Limestone Coast region.

This role offers the flexibility of casual employment while allowing you to build strong relationships within regional communities and deliver high-quality, evidence-based nursing care.

Key Responsibilities

  • Provide person-centred nursing care across community and home-based healthcare settings.
  • Assess client health needs and develop appropriate care plans to support positive health outcomes.
  • Coordinate healthcare services and collaborate with multidisciplinary teams to ensure continuity of care.
  • Deliver evidence-based nursing interventions while maintaining professional healthcare standards.
  • Provide health education and contribute to community health promotion initiatives.
  • Build strong relationships with clients, families, carers, and community partners.
  • Work independently while effectively managing competing priorities across community healthcare environments.

Skills & Requirements

  • Professional Registration: Registered or eligible for registration with the Nursing and Midwifery Board of Australia (NMBA) with a current practising certificate.
  • Clinical Experience: Demonstrated nursing experience with the ability to provide safe and evidence-based patient care.
  • Communication Skills: Strong communication, critical thinking, and problem-solving abilities.
  • Work Capability: Ability to work autonomously while collaborating effectively within a multidisciplinary team.
  • Community Nursing: Previous community nursing experience is desirable.
  • Driver Licence: Current South Australian Driver's Licence required.
